Senior Care Program

Shiprock Elder Care Program

Purpose: To provide high quality care to frail elders in a nurturing environment, including those diagnosed with Alzheimer's or related dementia, in order for elders to live in their homes and community longer to decrease the need for nursing home placement.

Eligibility for Participants

  • Be frail & 60 years of age or older.
  • Reside in Shiprock or surrounding area.
  • Be able to feed & toilet themselves with minimal assistance.
  • Not Exhibit Disruptive Behavior..
  • In Early Stages of Alzheimer's or Dementia.
  • Meet Intake and Assessment Criteria.

Service Description

Shiprock Elder Care Program staff is dedicated to providing services in a traditional, respectful and compassionate manner, we provide:

  • Continuous Supervision
  • Planned Social / Recreational activities
  • Arts and Crafts
  • Nutritious Morning / Afternoon Snacks
  • Lunch Provided at Shiprock Senior Center
  • Door to Door Transportation
  • Assistance with Medications
  • Health Screenings & Monitoring by Qualified Medical Professionals
  • Relief & Respite for Family Caregivers

Elderly Group Home Care Services

Purpose: To provide an independent, unrestricted home environment, to provide an alternative to institutional care, to expand services which are culturally appropiate.

Eligibility

  • Must be enrolled member of Navajo Nation
  • Certified eligible according to the Navajo Division of Social Services critera.
  • Applicant is ambulatory and requires twenty-four (24) hour care.

Service Description

The provisions of supervised residential custodial care to eligible clients for protection and personal care.
